PatientTrac · Clinical Network Mobile

Connect — your recovery, in your care team's sight between visits.

PatientTrac Connect is the patient mobile app of the PatientTrac Clinical Intelligence Network. Pair your iPhone with your care team by invitation and share your Apple Health data — read-only, and only with your care team.

Available for iPhone through a care-team invitation. Your data stays read-only and is shared only with your care team.

Built for trust, not tracking.

Connect does one job well: it carries the health signals you choose to share from your iPhone to the clinicians already caring for you — on the same secure clinical spine that powers every PatientTrac application.

🍏

Apple Health, read-only

Connect reads the Apple Health data you approve — it never writes to your health record on the phone, and you can revoke access at any time in iOS settings.

🔗

Paired by invitation

Pairing starts with a single-use code from your care team. No public sign-up, no browsing strangers' data — your device links to your chart and nothing else.

🩺

Only your care team

Readings are shared exclusively with the clinicians treating you, inside the same secure network that runs your practice's scheduling, EMR, and follow-up.

🌐

Part of the Network

Connect works hand-in-hand with PatientTrac Companion — your care plan between visits — and the PatientTrac Clinical Intelligence Network behind it.

Pairing takes three steps.

Your care team stays in control of who connects. Every pairing link is single-use; if one expires, just ask for a new invite.

Receive your invite

Your care team sends you a pairing link with a single-use code, like PT-XXXXXXXX.

Install & open

Download the app from the App Store if you don't have it yet, then open your pairing link on your iPhone.

Approve & sync

Approve the Apple Health permissions you're comfortable sharing, tap Sync — and your care team can follow your recovery between visits.

Ask your practice about PatientTrac Connect.

Connect is provided through practices on the PatientTrac Clinical Intelligence Network. If your clinic runs PatientTrac, ask your care team for a pairing invitation.
